To join the Dayton Underground Grotto, download the membership application and mail it, with dues, to:

Annual dues expire on May 31st of each year. Annual dues are:
- $20 Regular Membership
When renewing, please include the following information so your membership data is up to date:
- Full Name
- Address, City, State, Zip Code
- NSS Number (if applicable)
- Telephone Number(s)
- E-Mail Address
- Primary Grotto Affiliation
Send annual renewal dues, and the above information, to the following address:

Members who do not renew within 60 days of expiration will be dropped from the roles.
